Abstract

Hitnim pacijentima smatraju se svi životno ugroženi pacijenti koji zahtijevaju trenutačnu
stabilizaciju. Trijažiranjem hitne pacijente svrstavamo u četiri stupnja hitnosti, od kojih prvi
zahtjeva trenutačnu intervenciju, a posljednji stupanj intervenciju unutar 24 sata od nastanka
hitnog stanja. Trijaža pacijenata provodi se na temelju rezultata primarnog pregleda koji
obuhvaća procjenu prohodnosti dišnih puteva, samostalnog disanja pacijenta i postojanja
spontane cirkulacije. Ključno je unutar prvih 5 do 10 sekundi pregleda utvrditi nalazi li se
pacijent u kardiopulmonalnom arestu na koji sumnjamo kod svakog pacijenta s akutnim
gubitkom svijesti, uz promijenjeno disanje ili prestanak disanja. Kardiopulmonalni arest
indikacije je za trenutačno započinjanje kardiopulmonalne reanimacije čiji je cilj ponovna
uspostava srčane i plućne funkcije i osiguravanje dostatne perfuzije moždanog i srčanog tkiva.
Obuhvaća temeljne (ventilacija i masaža srca) i napredne mjere oživljavanja (monitoring
vitalnih funkcija, otvaranje venskog puta, administracija lijekova, terapija tekućinama, terapija
kisikom).

Abstract (english)

Emergency patients, by definition, are patients with life-threatening conditions that must be
immediately stabilized. By triaging, emergency patients are sorted into four stages of urgency
out of which the first stage demands immediate intervention and the fourth, an intervention
within the first 24 hours from the onset of the emergency state. The triage is based on the results
of the initial evaluation that includes the estimation of airway patency, independent ventilation,
and the presence of spontaneous circulation. During the first 5 to 10 seconds of initial
evaluation, it is crucial to determine whether the patient is in cardiopulmonary arrest, which
we suspect in every patient with acute loss of consciousness and respiratory distress or apnea.
Cardiopulmonary arrest is an indication for immediate cardiopulmonary resuscitation with the
purpose of restoring heart and lung function as well as securing adequate cerebral and coronary
tissue perfusion. It includes measures of basic (ventilation and chest compressions) and
advanced life support (monitoring vital functions, vascular access, drug administration, fluid
therapy, oxygen therapy)
