Abstract

Neoplazije predstavljaju jedan od najčešćih uzroka uginuća pasa i mačaka. Raspon u pasa iznosi od 18 - 23% sa većom incidencijom kod ženki. Među mačkama, neoplazije također često ugrožavaju život i kvalitetu života. Pod pojmom ketogene dijete podrazumijevamo unos visokog udjela masti, umjerenog udjela proteina te niskog udjela ugljikohidrata. Posljedično unosu makronutrijenata u navedenim udjelima, dolazi do promjena u energetskom metabolizmu: postiže se stanje ketoze i hipoglikemije. Stanje ketoze može se postići razdobljima posta ili smanjenim unosom tj. restrikcijom ugljikohidrata u prehrani. Novija istraživanja ukazuju da su neoplazije primarno metabolička bolest, obilježena promjenama u proizvodnji energije putem oksidativne fosforilacije i fermentacije te se smatra kako je moguće kontrolirati rast i razvoj tumora promjenom energetskog supstrata. Za razliku od normalnih stanica, tumorske stanice pokazuju povećanu potrošnju glukoze, čak i u anaerobnim uvjetima, što se najvjerojatnije događa zbog poremećaja u mitohondrijskom staničnom disanju pri čemu pojačana glikoliza predstavlja kompenzatorni odgovor. Stoga bi promjene u prehrani, poput ketogene dijete s visokim udjelom masti i niskim udjelom ugljikohidrata, koje potiču oksidativni metabolizam mitohondrija, istovremeno limitirajući potrošnju glukoze, predstavljale mogući nutritivni pristup selektivnom metaboličkom stresu tumorskih stanica. Primjena ketogene dijete kod pasa i mačaka u vidu hranidbenog režima tijekom liječenja ili oporavka kod onkoloških pacijenata, u postojećoj veterinarskoj znanstvenoj literaturi nije opisana. Dosadašnja istraživanja na životinjama dala su oprečne rezultate, što je najvjerojatnije posljedica korištenja različitih vrsta tumora, životinja i dijeta u istraživanju. Svakako je potrebo više kvalitetnih istraživanja na tu temu, posebice u pasa i mačaka. Nužan je sistematičan pristup u procjeni suplemenata, nutraceutika i novih prehrambenih režima koji će osigurati primjenu najprikladnijeg. Kako će biti dostupno sve više predkliničkih i kliničkih istraživanja, odabir hrane za onkološke pacijente trebao bi se proširiti i postati specifičan za određeni tip tumora i stadij bolesti

Abstract (english)

Cancer is one of the major causes of the death of dogs and cats. It causes 18-23% of death in dogs with even higher incidence in females. Among cats, it often endangers life or quality of life. Ketogenic diet consists of high fat, medium protein and low carbohydrate content. Consequently, such intake of macronutrients changes energy metabolism: ketosis and hypoglycemia is developed. Ketosis can be achieved by fasting or by reduction or restriction of the carbohydrate in the diet. Recent studies indicate that tumors are primarily a metabolic disease characterized by changes in energy production through oxidative phosphorylation and fermentation. It is also considered that their growth and development can be controlled by altering the energy substrate. Unlike normal cells, tumor cells have increased glucose consumption, even in anaerobic conditions, most likely due to mitochondrial dysfunction, with increased glycolysis as a compensatory response. Therefore dietary changes, such as ketogenic high fat and low carbohydrate diet, which induce oxidative metabolism, while limiting glucose consumption, represent a possible nutritional approach causing selective metabolic stress of tumor cells. The use of a ketogenic diet in dogs and cats as nutritional management during treatment or recovery in tumor patients has not been described in existing veterinary scientific literature. Previous research on animals has given the opposite results, most likely due to the use of different types of tumors, species and diets in research. Certainly more quality research is needed on this subject, especially in dogs and cats. A systematic evaluation of supplements, nutraceuticals and nutritional management is inevitable to ensure the most appropriate application. As more and more pre-clinical and clinical research is available, the selection of diet for tumor patients should become diverse and specific to a particular type of tumor and stage of disease
